Architectural Change Request

If you are considering a change to your property please complete the Architectural Change Request form. Your request will be reviewed by the Architectural Committee and a determination from the committee will be provided to you promptly. Homeowner agrees to be bound by the Community Rules and Standards set forth in the Declarations of Covenants, Conditions and Restrictions for Townhomes of Addison Homeowners’ Association, Inc.   • Please be sure to include detailed specifications (size/height/color/etc.) below, as well as a picture or drawing of your improvements and a plot plan detailing the location of the requested improvement(s) in order to expedite your request. Incomplete requests will not be submitted. IMPORTANT: Homeowner understands and agrees that no improvement shall commence prior to approval from the Architectural Committee. The homeowner also agrees by signing this document that all information submitted in this request is accurate and true.
